Substitute Breach Notification

Princeton HealthCare System (PHCS) values your decision to utilize our services for your health care needs. We regret to inform you that as part of a New Jersey State Police investigation, PHCS was notified on February 15, 2017 that a former employee of PHCS may have removed limited patient information from our premises. We have learned that protected health information (PHI) was copied and subsequently removed from PHCS by an employee during her employment between April 2007 and August 2012. The information included the name, date of birth, telephone number, address, and social security number of some of our patients. It did not include any medical record documents. PHCS is participating fully with the New Jersey State Police in this active and ongoing investigation, the details of which cannot be released at this time.

We have placed this notice on our website because we were unable to reach a small number of patients (less than 20) by mail, whose PHI was stolen as a result of this incident. PHCS takes the protection of patient information very seriously, and maintains a robust security program which is designed to monitor and prevent these types of incidents. However, despite these efforts, this employee, who is no longer employed by PHCS, violated our policies. If you were a PHCS patient during the time stated above and have any reason to believe your PHI may have been stolen or have any questions, please contact us at the number and/or email address listed below. Additionally, we have included the contact information of the three major credit reporting agencies if you wish to place a fraud alert on your credit report and/or receive a copy of your credit report free of charge.
